What a raw expression of longing and intense yearning. I've been there. The person in this poem has yet to get over his ex, this is why he or she feels so intensely still attached to the person who has set him or herself apart from the writer. The writer then bleeds until his wound is healed, leaving a scar. That's my interpretation. It's a great poem, very cut and dry, I almost want to look away in disgust because those feelings are so gnarly.
