Wow, your author notes are heart breaking. I'm so sorry for what you and your mom went through. It was the same with my grandma. It's a very difficult situation.
Knowing what I know from your notes, I would add more to your poem.
An example...
"Dementia robbed my sweet mother from me."
You can make changes until the contest begins. But even if you don't change it for the contest, you can do it after it's over too. I just would like to read more of your amazing story in the poem.
It's an exceptional entry for the Seven Words or Less Poem writing prompt contest. Good luck!

There's something absolutely debilitating and horrid about dementia, it robs them of their soul and is a mocker, I'm so glad you never resented her at any time. Good luck, blessings Roy
Typo: as this is a seven word contest, you might be disqualified for having only four words.

Dementia is sad. It is a disease which can make a person not be themself. In tribute of your mom, your poem says every word about this horrible disease. They can slow the disease down but is there a cure? God Bless You.

I like this poem a lot, the picture chosen is also well thought out. We must never forget about those whose world revolves in small places because of a disease of the mind and those of the body.

Firstly, I am sorry for your loss of you mom. This is a powerful entry in the "Seven Words or Less" poetry contest. The text is a good size. The message is clearly stated and easily understood. However, your message is weakened when the word dementia is misspelled everywhere except in your title. It's misspelled in the description above the title, in the poem and all through your AN.
The visual fits well. Good luck in the contest.

Indeed, the long good-bye that tests the bonds of our love and strength of our character. It is said that we are only truly known when we act out when we think no one is looking. Your taking care of your mom while it appeared to her that you were not there, speaks volumes of your love an commitment to her. Thank you for this post! Loren

Very good and compelling. I am sure this will garner a lot of attention and praise. I hate like hell the thought of being struck with this ailment. Thanks for sharing.

How very sad this is and you were able to rise above it all and still care for your Mother and love her even though all traces of her had disappeared. I feel for you and I am so sorry for your loss. I wish they could find a cure for this awful disease. Your few words speak volumes, love Dolly x

You chose the perfect photo for this, and squeezed a lot of story into very few words. I was distracted by the spelling of dementia. When I saw it as the title, I thought it made reference to the dimming of perceptions. The misspelling in your description made it just look like mispelling.

A succinct poem that covers an unfortunate problem. The succinctness echoes well the starkness of the condition. The presentation enhances the poem.
Suggestion:
The proper spelling is dementia according to internet searches.
