I am a fan of rhyming couplets and I like the story in poetic form you made with them. It is not always easy to have 100% end rhymes in this form.
Also, you did well with the presentation matching theme too.

This is a cute idea, writing the poem from the dog's point of view. Two small fixes: I will never forget the walks we (had); We may have (said) our last goodbye. I'd suggest working on your rhythm & length of lines to make them more consistent, for a higher score. So sorry for the loss of your pet. You chose the perfect picture for your words. I liked your poem. It reminded me of my boston terrier, Lainie. She's been gone for almost seven years now, but, as you said, her paw prints are forever on my heart. You really captured the relationship between a pet and their human. Keep up the good work.
