Debi, this gives me such a beautiful glimpse into your family and world! What a special treasure to have family so close and even more than that the love you all share. Which is no surprise when you are such a lovely and welcoming person Debi!! I would be so honoured to have a poem such as this written of me. The words are so heartfelt and honest. You have such a knack of story telling within your rhyme and meter it is just wonderful to read.

The image of the metaphor of "Bridge" is all encompassing in this account. You enhance the characters with sweet metaphors. This is very poignant. Nice expressed.

And as time went by, you'd complete us. I think you can capitalize grandma's in this line:
To grandma's, for playtime & more
That is just because it would be what the grandsons are calling you personally. But if you said "To their grandma's, for playtime and more" then you wouldn't need to capitalize it.

I feel like I'm getting to know your entire family and am so happy that they are close with you (and I imagine with each other). It's probably your heartfelt poems that bind your entire family together.
The MIL-DIL relationship is often tricky but it appears you (and Mel) have navigated it with grace and aplomb. And the bridge over the creek story is priceless. What a great "connecting" point for your family.
