Sometimes the chromosomes are awry (e.g. Kleinfelter's XXY Syndrome), or at odds with external appearance of the genitalia--e.g. Androgen Insensitivity (aka testicular feminization) and the converse (masculinization of the female fetus).

This is another funny one. There are a lot of people who refuse to acknowledge transgender people as who they have become. They will always be in some minds the gender they were born with.

It is hard to win with transgenders. It is hard to know how to refer to them, and each day, even, it can be different with certain ones (I think it's just the ones who want to be extremely difficult, though.)

This reminded me of a fun story. My BIL - who thinks he's pretty cool and rather smart - met a new couple at church. Later, (a different date) he was in a group of men gathered in the church foyer. One of he men happened to be the male of the couple he'd met. My BIL is - well, you'd have to know him.
He was occupying a lot of the convo and trying to bring in the new member. 'What do you think about that, Mr. Wolf?' 'That's the deacon we're referring to, Mr. Wolf.' 'They sit two rows behind you, Mr. Wolf.'
When Mr. Wolf walked away, all the other men turned to my BIL with wide eyes. 'WHAT are you DOING?' they asked. 'His name is FOX.'

Google refreshed my memory! I've been long fascinated by this subject.

There are a plethora of odd conditions that span the F/M spectrum. e.g. Intersex individuals have ambiguous genitalia--I believe that these days parents are advised to refrain from labeling and see which gender the child leans toward. In the old days they assigned one or the other at birth, which may turn out not to be the one the child identifies with.

We've talked about this before. I apologize for not remembering. It's kind of one of those things that doesn't make a difference to our relationship and that I didn't plan to ask a bunch of qs about.

A rose by any other name? A long-held belief of many cannot be rearranged. The transgender thing is to accommodate the person who feels the opposite of what he/she is. Those that observe haven't 'changed'.

Transgenders are an oddity because they swing between dressing as a man and a woman and it can be confusing for he neighbours.
